Remove (NEW) labels and feature toggle from VS Code extension

The old extension has been retired, so the (NEW) suffix is no longer needed
to distinguish between extensions. This removes:
- The feature toggle flag in constants.ts
- Four hardcoded (NEW) suffixes in package.json
- The coexistence documentation in AGENTS.md
