fix(vscode): invalidate cached ignore rules when ignore files change

Watch .kilocodeignore and .gitignore for changes, creations, and deletions.
When either file changes, the cached FileIgnoreController is disposed so the
next autocomplete request rebuilds it with fresh rules.
