Disable all GitHub Actions caching across workflows

Remove npm cache from actions/setup-node in changeset-converter,
smoke tests, and test workflows. Remove all explicit actions/cache
steps (node_modules, webview deps, VS Code, Playwright) from e2e
workflow.
