docs(readme): promote SDK into the surface grid as a peer

Move the SDK from a separate single-cell table at the bottom into the
main surface grid alongside CLI, VS Code, JetBrains, and Kanban. For
an SDK launch, the previous visual demotion sent the wrong signal.

New layout:
- Row 1: CLI + SDK (the two npm-installable launch surfaces)
- Row 2: VS Code Extension + JetBrains Plugin (IDE surfaces)
- Row 3: Kanban (centered via colspan=2)

The standalone single-cell SDK table is removed. The SDK cell copy was
shortened slightly to fit cell width parity with the CLI cell.
