Jax LiuandClaude Opus 4.6 a28058d4bb fix(wren): address CodeRabbit review feedback

- Guard queries.yml with --force in context init to prevent silent overwrite
- Drop _row_id column from memory list table display
- Warn (instead of silently swallowing) queries.yml load failures in memory index
- Validate YAML shape and add error handling for file read in memory load
- Deduplicate row_ids in forget_queries_by_ids to prevent inflated counts
- Collect all row ids per nl_query in upsert to remove every duplicate
- Fix doc wording to match main extra bundle install
