YANG QIA e03ec4dc6b chore(agent-runner): merge v3-develop

The runner now runs agents with full access and denies nothing, so
drop the policy refusal feedback this branch added: the hook denial,
the shared refusal text, Codex steer feedback and declined-result text,
and the Claude input lifecycle that kept the permission channel open.
Keep the too-old Claude Code reporting and the redacted denial log for
refusals a tool decides itself.
