* refactor(plugin-ai): make OrcaRouter HTTP-Referer/X-Title configurable
Remove hardcoded NocoBase attribution headers (HTTP-Referer / X-Title)
and expose them as optional provider settings instead. Headers are only
sent when the user fills them in, so NocoBase-based systems are no longer
forced to carry NocoBase attribution. Addresses review feedback.
