kiloconnect[bot] bcbd2b423b docs(vscode): prune completed items from migration plan

- Remove Settings Migration, Upgrade Onboarding, and MemoryBank Migration
  docs (all covered by the migration wizard shipped in #7823)
- Remove custom-openai-provider-ui.md (feature shipped)
- Update MCP docs: remove connection status and enable/disable toggle
  (shipped in #7810)
- Update settings-ui.md: remove import/export (shipped in #7794)
- Update opencode-migration-plan.md accordingly
