- Move d/h (deleted/hidden) control into history contents
- normalize names (show_deleted, showDeleted --> includeDeleted)
- add History.contentsShown that checks d/h flags to return num. shown
- move HistoryPreferences into HistoryContents.storage

Misc. fixes:
- fix collection.off to stopListening
- provide interface to HistoryPreferences so that consumers don't need
the model keys (easier to change them later on)
- (some) cleanup
