Workflow outputs logic rewrite. Accounts for old workflows that have no outputs defined but do use HideDatasetActions.

Fixes the bug where you couldnt' remove the last HideDatasetAction from a workflow in some situations.

Workflows containing tools that are not loaded will now open in the editor instead of failing on the dictionary lookup.
...
The offending nodes are marked as having an error state, and the workflow cannot be saved.
This will enable users to at least look at and try to recover a workflow by removing (and potentially replacing) the offending steps, as well as providing a better view of what's actually wrong with the workflow.

Shift management of the interaction between workflow outputs and HideDatasetActions to the front end editor.
...
This resolves the issue with multiple HideDatasetActions being created.
Existing workflows displaying multiple HideDatasetActions per step on the Run Workflow screen will persist.
These extra HideDatasetActions are harmless, but a simple edit workflow -> save will remove them.

Workflow changes-

Output flagging now has a third faded icon, and tooltip, and aligns properly on multiline outputs.
ToolForm width now properly calculated, taking into account the length of input and output rows.
-Max width is 250px, min is 150px.
Output extensions are now properly separated by ', ' (including space) instead of just slammed together.
Fixed a bug where a newly created node had no outputs array.

Enhance workflow editor to work with a list of output formats instead of only one. This fixes the bug were outputs utilizing a change_format tag could not be connected to an input that was of the changed-to type.

Numerous small changes as well: (a) added an 'edit workflow attributes' to workflow editor; (b) added 'Edit tags and Annotation' area to history panel; (c) added generic javascript method to ajax text updates.
