More work on eliminating references to test_db_util.sa_session.

... tweaked styling of several functional tests (the non-toolbox,datamanager,workflow,api functional tests) along the way - mostly PEP-8 fixes and eliminating * imports. Did not modify tests which do not pass - but did not in the source where they fail inside a TODO.

No longer allow Datasets to have DatasetPermissions set such that no roles are associated with the DATASET_MANAGE_PERMISSION.
...
Include the automatic creation of a new DatasetPermission for an hda where the DATASET_MANAGE_PERMISSION permission is associated with the hda.history.user's private role if the hda has no roles associated with the DATASET_MANAGE_PERMISSION permission. The creation of the DatasetPermission occurs when the user clicks the pencil icon for the hda.

First pass of the new sequencer_type & sequencer framework

- sequencer table now populated with the unique sequencer in the request_type table
- sequencer_type is introduced which defines a sequencer
- support for sequencer (create, delete, edit)
- added support for a simple sequencer for backward compatibility
- added support for Solid sequencer
- tweaked the request_type api methods. removed the sequencer login info.

Adding a 'name' attribute in the field dict for form_definitions. In the 'form_values'

table, the 'content' column is now a JSON dict instead of a list. Made appropriate code changes.

Code fixes and cleanup for adding and viewing sequencer configurations ( RequestTypes ). Move the request_type templates from the requests folder to their own request_type folder. Add the ability to edit sequencer configurations.

More miscellaneous bug fixes in Sample Tracking, all due to the recent UI changes. Changes the name of the test_forms_and_requests.py functional test script to be test_sample_tracking.py to more clearly reeflect what's being tested.
