wizardchen 4709bba488 fix(plugin): raise chat.answered for IM channel and MCP ask answers ...

chat.answered was only published by the web chat, so plugins never saw
answers given in IM channels or through the MCP server's ask tool. The
event is now built in one place, events.PublishAnswer, which the web
chat, the MCP ask tool and IM channels (through the IM service's OnAnswer
hook, set by the container) call once per completed answer. Cancelled or
failed runs raise nothing.
