wizardchen 1c555f7ac1 fix(plugin): store tool policies of plugin MCP services ...

Plugin MCP services are not rows of mcp_services, and mcp_tool_approvals
referenced that table, so enabling, disabling or requiring approval for a
plugin MCP tool failed with a foreign key error. The reference only
existed for a cascade that never ran, because MCP services are
soft-deleted. Versioned migration 000123 drops the foreign key and SQLite
migration 000041 rebuilds the table without it, so policies of plugin
services are stored and read like any other.
