fix(ui): label the Settings button in the header

Fixes #111 — the Settings entry point was a bare icon (a key glyph in
the Electron header, a gradient circle in the web shell), which users
mistook for an avatar or didn't notice at all. Replace both with a
gear icon + "Settings" text in a bordered pill button.
